Our US Family Health Plan is offered in Maine, New Hampshire, Vermont, upstate New York, northern and western Pennsylvania, and northeast Ohio.

For applicable in-network benefits, US Family Health Plan members may choose to receive care from over 25,000 providers across our service area (including all the major health care systems) who participate in our US Family Health Plan network.

**We’re expanding our network of civilian doctors and hospitals in Pennsylvania - including Lehigh Valley Health Network, University of Pittsburgh Medical Center and Allegheny Health Network, with more health systems being added! **

The provider directory is updated regularly, however, we recommend you call 1-877-757-7087 to confirm the selected provider is accepting new patients.
